Consent for Laboratory Testing Services

By using TotalVirility's services and proceeding with a blood draw, you acknowledge that you have read, understood, and agree to the following:

1. Nature of Services

I understand that TotalVirility provides laboratory testing services for informational purposes. These services include:

• Collection of blood samples by a licensed phlebotomist
• Laboratory analysis of blood samples
• Digital delivery of test results with explanations

2. Not Medical Advice

I understand that TotalVirility's services do not constitute medical advice, diagnosis, or treatment. The test results I receive are for informational purposes only and should not be used as a substitute for professional medical advice.

I agree to consult with a qualified healthcare provider for:

• Interpretation of my test results in the context of my overall health
• Any medical decisions or treatments
• Any concerns about my health or test results

3. Blood Draw Procedure

I consent to having blood drawn by a licensed phlebotomist. I understand that:

• The procedure involves inserting a needle into a vein, typically in the arm
• I may experience minor discomfort, bruising, or slight bleeding at the puncture site
• Rare complications may include fainting, infection, or nerve injury
• Multiple attempts may be necessary if veins are difficult to access

4. Preparation Requirements

I understand that some tests require specific preparation, such as:

• Fasting (not eating) for a specified period
• Morning blood draw for accurate hormone measurements
• Avoiding certain activities or substances

I agree to follow all preparation instructions provided to ensure accurate test results.

5. Identification

I agree to present valid government-issued identification at the time of my blood draw to verify my identity.

6. Test Results

I understand that:

• Test results will be delivered electronically to the email address I provide
• Results are typically available within 2-4 business days
• Some results may fall outside reference ranges, which does not necessarily indicate disease
• Reference ranges are general guidelines and may vary by laboratory

7. Limitations

I understand that:

• No laboratory test is 100% accurate
• False positive and false negative results are possible
• Results should be considered alongside other health information
• TotalVirility cannot guarantee specific health outcomes

8. Privacy and Confidentiality

I understand that my personal and health information will be handled in accordance with TotalVirility's Privacy Policy and applicable laws, including HIPAA.

9. Right to Withdraw

I understand that I may withdraw my consent at any time before my blood is drawn. Once the blood draw has occurred, I cannot withdraw consent for that specific test as lab processing begins immediately.

10. Questions

I have had the opportunity to ask questions about the testing process and have received satisfactory answers. I may contact TotalVirility at any time with additional questions.

11. Voluntary Consent

I am providing this consent voluntarily. I have not been pressured or coerced into using TotalVirility's services.
